Organizational Information

Legal Name and EIN – Exact name of the organization and Employer Identification Number.

Address and Contact Information – Principal address, phone number, and email.

Date of Incorporation and State of Incorporation – including articles of incorporation and bylaws.

Tax-Exempt Status Determination Letter – Copy of IRS determination letter confirming 501(c)(3) or other exempt status.

Fiscal Year End Date – Confirm the fiscal year for reporting. Typically aligns with the calendar year

Governing Documents – Bylaws, conflict-of-interest policy, and other relevant policies.

Financial Information

Statement of Revenue and Expenses – Detailed income and expense report for the fiscal year.

Balance Sheet – Assets, liabilities, and net assets at the beginning and end of the fiscal year.

Sources of Revenue – Breakdown of revenue from contributions, program service income, investment income, etc.

Expense Allocation – Classification of expenses into program services, management, and fundraising.

Program and Operational Information

Mission Statement and Program Descriptions – Overview of the organization’s mission and main programs.

Significant Activities – Details of major accomplishments and how they align with tax-exempt purposes.

Grants and Assistance Provided – List of grants given and support provided, if applicable.

Governance and Compliance

Board of Directors Information – Names, titles, compensation, and meeting minutes.

Key Employee Compensation – Salaries and benefits for officers, directors, and key employees.

Conflicts of Interest and Transactions – Any transactions between the organization and board members or key personnel.

Fundraising Activities – List of fundraising events and professional fundraiser contracts.

Additional Compliance and Reporting

Unrelated Business Income (UBI) Information – Any income generated from activities not directly related to the organizations mission or generated for profit should be explained in detail. If the amount of UBO is greater than $1000, Form 990-T must be completed.

Lobbying and Political Activities – If any lobbying was conducted, details of expenses and activities should be provided.

Foreign Activities – Grants, offices, or activities outside the U.S., including foreign bank accounts.
